This is a potentially decisive showdown for the top four, but Roma are decimated by injuries and bans as Andrea Belotti takes on Inter’s Romelu Lukaku.
It kicks off at the Stadio Olimpico at 17.00 UK time (16.00 GMT).
The Champions League battle could not be tighter, as going into this weekend there are only two points separating fourth-placed Inter from Atalanta, Milan and Roma.
Nemanja Matic returns from suspension, but otherwise it’s all bad news for Jose Mourinho, as Zeki Celik sits out a ban, with Stephan El Shaarawy, Chris Smalling, Diego Llorente, Marash Kumbulla and Rick Karsdorp injured.
Paulo Dybala and Gini Wijnaldum are at best fit for the bench, so Bryan Cristante has to drop back into the defence, with Ola Solbakken and Lorenzo Pellegrini supporting only striker Belotti.
It’s a surprise, as Belotti has a cracked rib and was not expected to feature, but is preferred to Tammy Abraham.
Inter make the trip without Robin Gosens, Danilo D’Ambrosio and Milan Skriniar after the 6-0 thrashing of Verona midweek.
Lautaro Martinez is still rested ahead of Wednesday’s Champions League semi-final against Milan, so Tucu Correa partners Lukaku upfront.
Inter are unbeaten at the Olimpico since a 2-1 result in October 2016, leaving here with two wins and three 2-2 draws.
However, the Giallorossi did win 2-1 at San Siro earlier this season.
