Rafael Leao scored twice in that game, his first goals since January, but is not 100 per cent fit and therefore rested along with Brahim Diaz and Olivier Giroud ahead of the Champions League quarter-final with Napoli.
Saelemaekers – who also found the net at the Stadio Maradona – steps in with Ante Rebic and Divock Origi, but Pierre Kalulu and Zlatan Ibrahimovic are still injured.
Empoli beat Lecce last week to end their eight-match winless streak and created a strong buffer to the relegation zone.
Lorenzo Tonelli is suspended, with goalkeeper Vicario still injured.
The Tuscans won their last trip to San Siro, beating Inter 1-0 on January 23 with a Tommaso Baldanzi strike.
