Charles De Ketelaere, Rafael Leao and Olivier Giroud are expected to lead the Milan attack, while Inter have decisions to make in defence, on the wings and to partner Lautaro Martinez.
The Derby della Madonnina kicks off on Saturday at 17.00 UK time (16.00 GMT).
Now that the transfer window is closed, the sides can finally focus only on the season ahead, which they hope will again see them tussle at the top of the table.
Milan have Divock Origi in the squad, but only fit for the bench, while Alessandro Florenzi, Rade Krunic and Zlatan Ibrahimovic are certainly out.
It remains to be seen if Ante Rebic will be able to shake off a knock in time.
The only real doubt for Stefano Pioli would appear to be in the supporting trio, where Junior Messias and Alexis Saelemaekers are fighting for a place.
Otherwise, it is De Ketelaere and Leao supporting Giroud.
Inter are missing Romelu Lukaku, but defender Alessandro Bastoni is a major doubt because he has been suffering from flu symptoms this week.
Federico Dimarco seems likely to play, either in the back three or wide left of the five-man midfield, while Edin Dzeko and Joaquin Correa are the options to partner Lautaro Martinez.
Correa had been the favourite, but as he has a slight flexor issue, the Bosnian is moving up.
Probable line-ups
Milan: Maignan; Calabria, Kalulu, Tomori, Theo Hernandez; Bennacer, Tonali; Messias, De Ketelaere, Leao; Giroud
Inter: Handanovic; Skriniar, De Vrij, Bastoni; Dumfries, Barella, Brozovic, Calhanoglu, Darmian; Lautaro Martinez, Dzeko
Wow 4 Italians on the field
The day when Inter have more Italians in field than Milan.
Sad to see the lack of Italians playing on two of our biggest teams. Italian football is in disarray to say the least! No one the national team is on a downward tail spin.
I really liked to see Lukaku starting for Inter to see how the play has changed compared to last season but what happened happened. Glad to see Milan playing their best 11 otherwise we had to get ready for the usuall excuses. Forza Inter
Basically the same teams as last year, with Inter losing Perisic and Milan losing Kessié while adding De Ketelaere.
“Federico Dimarco seems likely to play, either in the back three or wide left of the five-man midfield”
FI, where he will play?
Inter may win the game but Inter are always physicality and stupidity. AC Milan are about flair and grace.
*5 Italians…
