The Colombian is now expected to undergo a medical with OM next week, prior to putting pen to paper at Stade Velodrome.
Bacca joined Milan from Sevilla for €30m in 2015 and scored 34 goals for the Rossoneri, but the 30-year-old fell out of favour last season.
