It kicks off at 19.45 GMT and the winners go on to face Lazio in the two-legged semi-final next month.

Both sides go in to this unusual Derby della Madonnina off two consecutive Serie A defeats that have seen them slide down the standings.

The Rossoneri are in particularly dire straits and winning the Coppa might be their best chance of qualifying for Europe next season.

Coach Gennaro Gattuso described this game as “a World Cup Final” for Milan, but he has to do without goalkeeper Gigio Donnarumma, who picked up a minor upper thigh strain in training and is also at risk for Saturday’s Serie A clash with Fiorentina.

Veteran Marco Storari therefore takes the gloves for this big game, but Donnarumma insisted on travelling with the team to the stadium.

However, the man who turns 41 next month pulled out with a calf injury in the warm-up, so Antonio Donnarumma – Gigio's older brother – makes his first senior appearance in a Milan jersey.

Suso and Alessio Romagnoli return, having missed the 2-0 defeat to Atalanta through suspension, with Lucas Biglia chosen to pull the midfield strings.

Inter were unbeaten in all competition until a couple of weeks ago, but have now lost two on the bounce to Udinese and Sassuolo, dropping them down to third in Serie A.

Samir Handanovic was originally expected to be rested, but he retains the gloves, while Andrea Ranocchia and Yuto Nagatomo come in for injured Joao Miranda and Danilo D’Ambrosio.

Joao Mario is given the nod rather than Borja Valero.

These sides have met four times in the Coppa Italia quarter-finals, with Milan progressing on the first two occasions and Inter the latter two.

The last four Milan Derbies in all competition have provided 16 goals.

Over 50,000 fans are expected at San Siro this evening.
