It kicks off at the Stadio Olimpico at 17.00 UK time (16.00 GMT).
These two teams are locked together in joint fourth place on 56 points and they drew 2-2 back in January, so this result could be absolutely crucial in the race for Champions League qualification.
If they finish level on points, then head-to-head record will be used to divide them.
Roma are ravaged by injuries ruling out Chris Smalling, Diego Llorente, Rick Karsdorp and Gini Wijnaldum, while Dybala is only fit for the bench after spraining his ankle in the 3-1 defeat to Atalanta.
Andrea Belotti and Tammy Abraham start in an experimental attack with two centre-forwards.
Milan make the trip without Alessandro Florenzi, Tommaso Pobega and Zlatan Ibrahimovic, but Davide Calabria returns from suspension.
Giroud is also back after missing the win over Lecce with a knock, supported by Rafael Leao and Brahim Diaz.
The Rossoneri are unbeaten against the Giallorossi since a 2-1 result here in October 2019, followed by four wins and two draws.
