According to the Gazzetta dello Sport, the Dutch international could be heading for Stamford Bridge over the summer.

The 26-year-old is wanted by potential new Chelsea manager Guus Hiddink and available for approximately £21m.

Sneijder had already been linked with a Premier League move to Manchester United.
