Simy had equalised twice from Crotone's first two efforts on target, cancelling out first-half goals from Milinkovic-Savic and Luis Alberto.
But when it seemed to end with a draw at the Olimpico, substitute Caicedo stopped Gonzalo Escalante's finish and perfectly timed his finish to give Lazio the winner five minutes from full time.
