He turns 24 today and will be celebrating on Italian soil ahead of his transfer.
Sky Sport Italia claim Krejci arrived today and will have his medical on Wednesday.
He was under contract with Sparta Prague until June 2017, having come up through their youth academy.
The cost of the operation is believed to be around €4m.
Krejci already has 26 senior caps for the Czech Republic, scoring three goals.
