The Giallorossi scraped a 2-1 win away to Fiorentina midweek, but lost Jordan Veretout for at least a month to a thigh strain.
Also still out of action are suspended Marash Kumbulla, injured Edin Dzeko, Roger Ibanez, Juan Jesus, Davide Santon, Riccardo Calafiori and Nicolò Zaniolo.
Smalling and Pedro return to the starting XI, with Amadou Diawara filling in for Veretout in midfield, leaving El Shaarawy to support Borja Mayoral.
