Claudio Ranieri’s Cagliari can only be promoted to Serie A with a victory at the Stadio San Nicola, otherwise Bari will make their return to the top flight.
The promotion play-off starts at 19.30 UK time (18.30 GMT).
The first leg ended 1-1 in Sardinia with a last-gasp Bari equaliser that puts them in pole position to join Frosinone and Genoa in earning promotion from Serie B.
This is because they finished higher up the table, so if the tie is level on aggregate, Bari are promoted without extra time or penalties.
The Stadio San Nicola is packed out with over 58,000 spectators.
Bari captain Valerio Di Cesare pulled out in the warm-up in the first leg, but returns to the defence here just weeks after turning 40, with Morocco international Walid Cheddira upfront.
Boris Radunovic performed heroics last time in goal for Cagliari, while striker Gianluca Lapadula leads the attack.
