OR
- Milan win by one clear goal, but concede at least one goal, and Atletico Madrid win by two clear goals (while conceding), due to Atleti having more away goals.
OR
- If Milan win and Atletico Madrid win in a way that allows them to have a superior overall goal difference.
Porto qualify if…
- They beat Atletico Madrid.
