The Dutchman has six months left on his contract with the Premier League side, and The Sun reports that Juve have offered the winger £30,000 a week to sign for the Italian champions.
Chong has only made two appearances in the Premier League this season, but has also played in the Europa League and the FA Cup.
The winger signed from Feyenoord in the summer of 2016.
