The midfielder will return to his former outfit on a Bosman free transfer given that his Rossoneri contract expires in June.
Milan and PSV will only confirm the deal once the season is over.
The former Barcelona player joined Milan in January 2011 on a six-month agreement from Bayern Munich, before being handed a new one-year contract last summer.
The player spent six years at PSV before his 2005 switch to Barcelona.
Van Bommel has played in 33 games across all competitions for the Diavolo this term.
