The 25-year-old is no stranger to Italian shores, having played for Fiorentina between 2013 and 2018.
He made just 12 appearances for the Viola during that time, but they are entitled to half of the fee the Diavolo could pay for his services in 2021.
Milan turned to Rebic after they failed to agree a fee for Atletico Madrid’s Angel Correa.
In return, the Bundesliga side are receiving Silva on the same basis.
“The clubs have reached an agreement,” said their sporting director Fredi Bobic.
“If all goes well, Silva will become an Eintracht player and Rebic will go to Milan.”
The former Porto striker had been left in limbo as he saw a proposed transfer to Monaco break down over a medical.
