Perotti arrived from Genoa in July 2016 and has played 138 games for the club, struggling with several injuries since his arrival in Rome.
Al-Shabab have reportedly offered the player a salary of €2.5m a year but Perotti wants them to match his current wages at Roma, worth €3m a season.
If he moves to Al-Shabab, Perotti will find his compatriot Ever Banega there. The former Inter player signed with the Saudi Arabian club in January.
