The Biancocelesti have won their last two games, and are unbeaten since a 1-0 loss to Inter at the end of September.
"It is a great opportunity for us, it is a game we have to play well in and stay focused for 95 minutes," Leiva told Lazio TV.
"Milan have a good team and having changed coach, they will have more motivation, but we've come from two great victories in the past week and we want to remain on this path."
Lazio haven't beaten Milan at San Siro in Serie A in 30 years, with their last win coming at the beginning of the 1989-90 season.
However, there is plenty of motivation, as a victory would put Lazio into joint fourth place with Atalanta and Cagliari.
