Lorenzo Insigne, Victor Osimhen, Mario Rui, Kalidou Koulibaly and Fabian Ruiz are out of Napoli’s trip to Milan.
It kicks off on Sunday at 19.45 GMT at San Siro.
The Partenopei are again decimated by injuries and the crisis is only getting worse for coach Luciano Spalletti.
Insigne has not recovered and is not even in the squad for this journey to Milan.
Osimhen had hoped to make a comeback with the aid of a protective mask after surgery on multiple facial fractures, which also leaves his Africa Cup of Nations participation in doubt.
Mario Rui is the latest addition to the absentee list, although Stanislav Lobotka and Eljif Elmas are available after knocks.
It therefore looks as if Spalletti might re-route Giovanni Di Lorenzo to left-back, installing Kevin Malcuit on the right.
Napoli squad for Milan: Marfella, Meret, Ospina; Di Lorenzo, Ghoulam, Juan Jesus, Malcuit, Rrahmani, Zanoli; Anguissa, Demme, Elmas, Lobotka, Zielinski; Lozano, Mertens, Ounas, Petagna, Politano
