The first leg ended 1-0 with a Brahim Diaz goal at San Siro, so as away goals no longer count double, any Tottenham win by a one-goal margin would send this Round of 16 tie to extra time.
Spurs need to score at least twice to progress without penalties, but have done that only four times in their last 12 matches.
It could be Milan’s first entry into the Champions League quarter-finals since 2012.
There were some doubts over Brahim Diaz’s fitness after he skipped the defeat to Fiorentina, but he is available and Giroud has also shaken off flu symptoms.
Leao and Rade Krunic were suspended for that 2-1 loss in Florence, while Ismael Bennacer is only ready for the bench after his return from a long-term injury.
Conte’s last game on the Tottenham bench was that first leg at San Siro on February 14, as he rushed his return from emergency gallbladder surgery and was ordered by doctors to take more time off to fully heal.
He returns tonight, but Hugo Lloris, Rodrigo Bentancur, Ryan Sessegnon and Yves Bissouma are still out of action.
Eric Dier sits out a ban, but Pierre-Emil Hojbjerg is back after his suspension in the first leg.
